A native of Upper Moreland, Pennsylvania, Major Jeffrey P. Toczylowski died 3 November 2005, when he fell from a helicopter during a combat infiltration in Al Anbar Province, Iraq, during Operation IRAQI FREEDOM.

Graduating from Valley Forge Military Academy, Toczylowski was commissioned as a Military Police Officer in 1995 and was assigned to the 551st Military Police Company, Vaihingen, Germany. In 2003, he joined the staff of Special Operations Command, Europe. He graduated from the Special Forces Qualification Course at Fort Bragg, North Carolina, in 2003.

MAJ Toczylowski was assigned as a Detachment Commander to 1st Battalion, 10th Special Forces Group, Stuttgart, Germany. He was posthumously promoted to Major.
