A native of Alexandria, Virginia, Captain Shane R. M. Mahaffee died on 15 May 2006 from wounds sustained while supporting Operation IRAQI FREEDOM.

Mahaffee earned his commission in 1991 through the ROTC program at Southwest Missouri State University. While serving in the 84th Infantry Division, U.S. Army Reserve, he completed his Juris Doctor degree from Widener University in 1994. In 2006, he graduated from the Civil Affairs Qualification Course at Fort Bragg, North Carolina.

CPT Mahaffee was assigned to 489th Civil Affairs Battalion, Knoxville, Tennessee, when an improvised explosive device detonated near his vehicle while on combat patrol in Al Hillah, Iraq.
